A productive appointment will require an exchange of information. A medical history and an examination will be necessary to determine your needs and formulate a treatment plan. Forms and notices on a first appointment are unfortunately necessary. Some forms are needed to provide you with the best health care available; others are required by your insurance company, state law, or federal law. You may complete these forms when you come to the office, but it may be more convenient to print and complete them prior to your arrival.
